Medicare Enrollment Timing: Get It Wrong and Pay for Life

Medicare enrollment has strict deadlines with permanent financial penalties for missing them. The rules are confusing, and the government does not send you a reminder. Here is everything you need to know to enroll at the right time.
Initial Enrollment Period (IEP)
Your IEP is the 7-month window around your 65th birthday: 3 months before, the month of, and 3 months after. Enrolling during the first 3 months means coverage starts on your birthday month. Enrolling after your birthday month delays coverage.
Special Enrollment Period (SEP)
If you have employer health insurance when you turn 65, you can delay Medicare without penalty. You have an 8-month SEP after your employer coverage ends to enroll in Part B without penalty. COBRA does not count as employer coverage for this purpose.
Late Enrollment Penalties
The Part B late enrollment penalty is 10% of the standard premium for each 12-month period you were eligible but did not enroll. This penalty is permanent β you pay it for the rest of your life. The Part D late enrollment penalty is 1% of the national base beneficiary premium for each month you went without creditable drug coverage.
The Right Strategy
If you have employer coverage from an employer with 20 or more employees, you can safely delay Medicare. If your employer has fewer than 20 employees, Medicare becomes primary and you should enroll. When in doubt, consult a licensed Medicare specialist before your 65th birthday.
